An ‘exacerbation’ means either an asthma attack or a sudden worsening in your child’s asthma where you needed to increase their inhalers. They may also have needed steroid tablets, nebulisers and/or antibiotics.

It is essential to have a good inhaler technique to ensure that your child’s medication gets to the part of their lungs that need it. Please watch the specific inhaler videos below to check that you/they are using their inhalers correctly:

How is your asthma today?
How much of a problem is your asthma when you run, exercise or play sports?
Do you cough because of your asthma?
Do you wake up during the night because of your asthma?

As the person completing the form, please answer the following 3 questions on your own:

During the last 4 weeks, how many days did your child have any daytime asthma symptoms?
During the last 4 weeks, how many days did your child wheeze during the day because of asthma?
During the last 4 weeks, how many days did your child wake up during the night because of asthma?
