Use this service to temporarily register with the surgery.
You can use this service if you:
- want to register at the surgery temporarily
- if a staff member at this practice has advised you to do so
If you need a repeat prescription you should contact your own Medical Practice who can send an electronic prescription to a local pharmacy. You will need to tell your Medical Practice the full postal address of where you would like your prescription to be sent.
If you have clinical problem you should contact your own GP by telephone 1st for advice as they hold your full medical records.
If your own GP advises that you need a Face to Face consultation telephone our reception team and we will speak with our Duty Clinician for advice and arrange appropriate actions.
Before you start
We’ll ask you for:
- your first and last name, date of birth, sex, postcode, email and phone number
- if applicable, the details of the person you are completing the form on behalf of
- your NHS number
- details of previous GP surgery
- basic health and medical information
